i am sorry mr wan i did not pass up the consent form because i misplaced it.when u call me n tell whether i got the form i should have check before replying to you i am really remorseful but i promised this will be the last time also i apolosise for not doing my correction before passing up to u i should have checked first . i am sorry that i have wasted a lot of your time but next time i will checked before passing up to you i hope you can give me a chance
